Who this covers

This policy describes how Infinity Health & Wellness LLC ("we", "us"), operating as Infinity Health and Wellness, handles information collected through this appointment request page. The page is operated on our behalf by Digital Wellness Partners.

It covers information you give us on this page only. Information you give us as a patient of the clinic, once care begins, is handled under the clinic’s own patient privacy notice.

What we collect

The answers you select on this page, and the name, phone number and email address you enter. We also record standard technical information such as your approximate location from your IP address, the device and browser you used, and the advertisement or link that brought you here.

We do not ask for, and you should not enter, medical records, diagnoses, insurance identifiers or any other sensitive health information on this page.

Why we collect it

To contact you about the appointment you requested, to confirm a time, to answer questions, and to keep a record of your request and of the consent you gave.

We also measure how well our advertising works. Advertising platforms may receive a coded, non-readable version of your email address or phone number so they can tell that an advertisement led to a request. They do not receive your answers on this page.

Calls and text messages

If you tick the box on the form, we may contact you about your request by text message and by telephone, including calls placed with an automated or AI-generated voice. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop messages, or HELP for help.

An automated call will say at the start that it is an automated assistant, who it is calling for, and the number you can call back. Tell it to stop calling, at any point, and it will end the call and add your number to our internal do-not-call list. We place calls only between 8am and 9pm in your local time.

If you do not tick that box we will not text you and will not place automated calls. A member of the clinic team will call you instead.

No mobile information will be sold or shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors in support services, such as our customer relationship management, telephony and messaging providers, is permitted. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Call recordings

Calls to and from this clinic may be recorded and transcribed so we have an accurate record of what was agreed and can improve how we handle enquiries. If you would prefer not to be recorded, say so at the start of the call and we will note it.

Who we share it with

Our service providers, and only so they can do their job for us: the customer relationship management system that stores your request, the messaging and email providers that deliver our replies, and the advertising platforms described above.

We do not sell your personal information. We may disclose information if the law requires it, or to protect the safety of a person.

How long we keep it

We keep appointment requests and consent records for as long as we need them for the purposes above and to meet our record-keeping obligations, and then we delete them.

Your choices

You can ask us for a copy of the information we hold about you, ask us to correct it, or ask us to delete it. Reply STOP to any text message to stop texts, say “stop calling” on any call to stop calls, or use the unsubscribe link in any email to stop emails. Stopping messages does not withdraw a request you have already made; call us if you want the request cancelled.

Arizona residents and residents of other states may have additional rights under state law. Contact us using the details below and we will honour any right that applies to you.

Children

This page is not directed at anyone under 18 and we do not knowingly collect information from children.
